What to expect from
your first visit.

No surprises. No pressure. A first visit is a conversation about what’s going on and what might help. Here’s how it works.

Request your appointment

Use the form on our Contact page, call us at (541) 200–2777, or send us a message through the patient portal if you’re already a patient. Tell us what you’re experiencing and what you’re hoping to address. We’ll confirm your appointment within one business day.

Complete your intake paperwork

Before your visit, you’ll receive intake paperwork through the patient portal. This covers your health history, current medications, and what brings you in. Completing it in advance means we spend your appointment time on you, not on forms.

Your first appointment

Your provider will review your history, ask questions, and examine your concerns. For dermatology visits, this typically includes a skin assessment. For wellness visits, it may include a broader health conversation. We move at your pace — and we explain what we’re seeing as we go.

Your care plan

At the end of your visit, your provider will walk you through a recommended plan — which services apply, in what order, and why. You won’t be pressured into anything. If a phased approach makes sense, we’ll say so. If a referral elsewhere is the right call, we’ll make it.

Follow-up and ongoing care

Follow-up appointments are scheduled based on your care plan. Many follow-up visits for dermatology can be done via teledermatology — especially for medication reviews and condition check-ins. Your patient portal is available 24/7 for messages, results, and refill requests between visits.

Before you arrive

What to bring.

Photo ID

A government-issued photo ID is required for all new patients.

Insurance card(s)

Bring your current insurance card. See our Insurance page for coverage details.

List of current medications

Include prescription medications, supplements, and any topical products you currently use.

Medical records (if relevant)

Prior dermatology records, lab work, or imaging — especially if you’ve been evaluated elsewhere for the same concern.

Questions for your provider

Write them down. We’d rather answer them all than have you leave with something unaddressed.

Can’t come in?

Teledermatology visits.

If you’re located outside the Medford area — or if you’d simply prefer a video visit — teledermatology is available for many dermatology consultations, follow-ups, and prescription reviews.

Teledermatology visits are conducted through our secure patient portal and meet the same clinical standards as in-person appointments. When an in-person exam is genuinely needed, we’ll tell you before you book.
